Technical Assistance:
Discussion of the change of location for the Family Day Home (FDH). The new address is 3930 Kernstown Court, Fairfax, Virginia 22033. The layout of the new home is identical to the layout and space of the old residence (the two homes are actually located in the same development). The by-right number set for capacity is 5. The Provider expressed the desire to submit a special exception application to the Fairfax Zoning office to increase this capacity to a higher number. Once this application has been approved, the Provider will submit the approved documentation to the Division of Licensing in order to modify the license again at that time.

Comments:
The following inspection took place between the hours of 9 am through 10 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced non-mandated, monitoring inspection there were no children in care at the new address. The residential part of the home is still being set up. The child care area was set up and was observed for compliance. The new Emergency Preparedness and Response Plan form was also reviewed. The current license shall be modified in order to reflect this change of location. If you should have any future questions, please call or send an e-mail. Thank you. Charlie Perkins, Licensing Inspector (703) 309-3963
