Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:15am through 12:30pm with the provider on site. There were 5 children (15 points) with the provider and volunteer. A sample of 5 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: taking a walk outside and playing in the backyard. Interactions between the children and caregivers were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-111-880
Description: Based on review of the menu, the family day homes did not follow the most recent, age-appropriate nutritional requirements of a recognized authority. Evidence: 1. Tuesday's lunch was listed as Rice, Beans ,fruit and milk. 2. Thursday's lunch was listed as Rice, Beans and Apple.

Plan of Correction: I will make sure the menu list the vegetables given.

Standard #: 22VAC40-111-980-A
Description: Based on record review, the provider did not obtain written permission from the parent of each child for the provider to take the child off the premises of the family day home. Evidence: 1. Child #2, Child #3, Child #4 and Child #5 were observed taking a walk with one of the caregivers during the inspection off the premises. The caregiver stated she could not locate the permission slips.

Plan of Correction: I will look for the permission slips or have the parents fill them out.

Standard #: 22VAC40-111-90-C
Description: Based on record review, the family day home did not obtain documentation of additional immunizations for a child once every six months for children under the age of two years. Evidence: 1. Child #5 had an immunization record dated 9/13/2018 and had not been updated every 6 months and was under the age of two years old.

Plan of Correction: I will ask the parent for an updated immunization record.

Standard #: 22VAC40-111-210-A
Description: Based on review, the caregivers did not obtain a minimum of 16 hours of training annually in areas relevant to their job responsibilities. Evidence: 1. Caregiver #1 had documentation of 14 hours of training. 2. Caregiver #2 did not have documentation of training.

Plan of Correction: I will locate the training certificates and I have signed up for classes.

Standard #: 22VAC40-80-120-E-1
Description: Based on observation and interview, the provider did not post the most recently issued license. Evidence: 1. The provider stated she could not locate the most recently issued license.

Plan of Correction: I will get a copy of the license to post.
