Comments:
An unannounced renewal inspection was conducted today between 1:45 pm and 3:30 pm. There were 9 children (25 points) with 2 caregivers. The physical plant, children's records, caregiver's records, household members records, caregiver training, menu's, attendance logs, evacuation drill logs, emergency plans and emergency supplies were inspected. The children were observed napping, waking up, reading books, infants were also fed during the inspection. Areas of non compliance were cited on the Violation Notice. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on children's record review, provider did not maintain up to date records for three children enrolled.

Evidence:
1) Child #2's record does not contain addresses for either of the two listed emergency contacts and the physical form was not signed by the parent.
2) Child #3's record does not contain the address for one of the emergency contacts.
3) Child #5's record does not contain the name and policy number for medical insurance.

Plan of Correction: I will get the information from parents and place in the children's files.

Standard #: 22VAC40-111-180-A
Description: Based on a review of household member's records, the provider failed to ensure that each adult household member will have a current Report of Tuberculosis Screening form every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.

Evidence:
Household member #1 does not have a current TB Screening form, the last date of screening was 01/30/16.

Plan of Correction: I will have member to get TB screening.
