Technical Assistance:
Technical assistance was providered regarding the out of state background checks. On the DSS website, there is a link to Adam Walsh out of state background checks. Look for the information for the 2 states you need, print the form and follow the directions. Send the inspector documentation that they were sent in within 10 business days, by 10/24/19 and when the results are received back within 30 business days 11/25/19.

Comments:
An unannounced renewal inspection was conducted today from 9:00am-11:00am. There were 7 children (20 points) supervised by 2 direct caregivers. The home, 2 staff records, 7 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaged in circle time, singing and interacting with the caregivers. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-750-A
Description: Based on observation, 1 out of 8 topical skin products are not labeled with child's name.
Evidence:
A diaper cream was labled with a child's name.

Plan of Correction: I will make sure names are on all creams.

Standard #: 22VAC40-111-1020-G
Description: Based on observation and interview with provider, towels are bring used to dry hands after handwashing.
Evidence:
Cloth hand towels are located in the bathroom the children use, no single use towels are observed.

Plan of Correction: I will begin using single use disposable towels after handwashing.

Standard #: 22VAC40-111-100-B
Description: Based on review of 7 children's records, 1 is incomplete.
Evidence:
Child #1, start date 7/8/19, does not have documentation of a physical examination.

Plan of Correction: I will get a copy of her physical and put it in her record, going forward I will make sure to have it with 30 days of their start date.

Standard #: 22VAC40-111-250-A
Description: Based on observation, a hanging item was accessible to children under five years of age.
Evidence:
A window blind cord was hanging and within reach of children under the age of five years old.

Plan of Correction: Corrected during inspection, it just fell.

Standard #: 63.2-1720.1-B-3
Description: Based on review of 2 staff records, 1 does not have documentation of an out of state background check for a state they lived in within the past five years.
Evidence:
Staff #1, date of hire 4/19, have lived out of state, in 2 different states, within the past five years and does not have documentation of an out of state background check search.

Plan of Correction: I will send them out and send you a copy of the documentation within 10 days and then when the results come in.
