Comments:

An unannounced monitoring inspection was conducted today approximately 1:30 pm-3:15 pm. There were 4 children, totaling 10 points, supervised directly by the provider. The physical plant, 5 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs at this time. The children were observed napping and story reading. Description:	Based on observation, the licensee failed to ensure that all children's records were keep confidential. Evidence: 1. The inspector observed that the supplemental page (dated 7/26/2019) that contains the children's name from the previous inspection was posted on the parent board. Supplemental page states that it is not to be posted or not for release.

Description:	Based on record review of 3 out 4 children?s records reviewed did not have the required information. Evidence: 1.
