Technical Assistance:
The Tuberculosis screening for Household member 2 is expiring soon (3/21/2020).

Comments:
An unannounced monitoring inspection was conducted today from 9:30am-11:00am. There were 6 children (12 points) directly supervised by the provider. The childcare area, 1 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed participating in circle time and talking about hibernation, painting cardboard car garages, watching an animated book reading about hibernation, and playing freely. The provider was observed encouraging children to get along and facilitating the children cleaning up toys between activities. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, the provider did not review the emergency plan at least annually and update the plan as needed.
Evidence:
The most recent documented review of the emergency preparedness plan was conducted on September 23, 2018.

Plan of Correction: I will review the emergency plan.

Standard #: 22VAC40-111-60-A
Description: Based on record review and provider statement, the provider did not maintain an up-to-date record for each child in care.
Evidence:
Child 6 did not have a record availible for review on the date of inspection.

Plan of Correction: The child has just enrolled and the parents still have all of the paperwork. I will request that they return the completed forms as soon as possible.

Standard #: 22VAC40-111-70-A
Description: Based on a review of children's records, 4 children's records did not contain documentation that parents have been provided in writing all required policies of the family day home.
Evidence:
The records for Child 1, 3, and 6 did not contain documentation that parents have been provided in writing information about the following policies of the family day home: : Operating information including the hours and days of operation, holidays or other times closed, and the telephone number where a message can be left for a caregiver; Schedule of fees and payment plans; Check in and check out procedures; Requirement for the family day home to notify the parent when the child becomes ill and for the parent to arrange to have the child picked up as soon as possible if so requested by the home; Requirement for the parent to inform the family day home within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life-threatening diseases, which must be reported immediately; Requirement for the child to be adequately immunized as required by 22VAC-40-111-90; Requirement for paid caregivers to report suspected child abuse or neglect according to ? 63.2-1509 of the Code of Virginia; Custodial parent's right to be admitted to the family day home any time the child is in care as required by ? 63.2-1813 of the Code of Virginia; General daily schedule that is appropriate for the age of the enrolling child; Policies for the provision of food; Discipline policies including acceptable and unacceptable discipline measures; Presence of a pet or animal in the home; Amount of time per week that an adult assistant or substitute provider instead of the provider is scheduled to care for the child and the name of the adult assistant or substitute provider; Parental notifications required in 22 VAC 40-111-650; Policies for termination of care; and Address of the website of the department, with a note that a copy of this regulation and additional information about the family day home may be obtained from the website, including compliance history that includes information after July 1, 2003.

Plan of Correction: I will ensure that parent receive the information on the policies.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation that one child in care has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
Child 5 did not have documentation of immunizations at the family day home on the date of inspection.

Plan of Correction: I will ask the parents to provide the immunizations.

Standard #: 22VAC40-111-210-A
Description: Based on record review, one caregiver did not obtain sixteen hours of annual training.
Evidence:
The record for Adult 1 contained documentation that 14 training hours were completed in the previous year.

Plan of Correction: I have signed up for training to complete in the near future.

Standard #: 22VAC40-111-280
Description: Based on observation, not all potentially poisonous substances were stored in areas inaccessible to children.
Evidence:
A bottle of spay cleaner and a canister of air freshener were observed on a low shelf in a room accessible to children.

Plan of Correction: I will move the substances to an area that is inaccessible.

Standard #: 22VAC40-665-450-D
Description: Based on record review, the provider did not maintain documentation of current rabies vaccination for dags present in the family day home.
Evidence:
There was no documentation of the rabies vaccination for Dog 1.

Plan of Correction: The dog is not always present and has received the vaccination. I will obtain the record.
