Technical Assistance:
Consultation on forms for permission to transport be filled out consistently between children, completing the Information For Parents (making copies) and ensuring parents sign the forms. Discussed safety issues during the monitoring visit (during diapering, crib safety and choking hazards). If the Licensing Inspector observes soft bedding in a child's crib, in the future, an adverse enforcement action will be recommended.

Comments:
An unannounced monitoring visit was conducted from 10:00-11:30. Five children (15 points) were present with the provider. Observed infant care, free play, physical space, emergency preparation, records and am snack. The Licensing Inspector went back on 2/12/2020 to review documents and added to a violation. Description: Based on observation, a high chair tray was not sanitized between uses.
Evidence:
The Licensing Inspector observed two different children sitting in the high chair. The tray was not sanitized between use by each child.

The provider currently only has 2 cribs set-up. The Inspector observed that the youngest child was sleeping in a bouncy seat and not a crib. The provider stated that the child regularly sleeps in the bouncy seat.

Description: Based on observation, soft bedding was in cribs while occupied by children.
1. Child #2 was observed on 2/6/2020 being placed on a decorative pillow in the crib to go to sleep. The pillow was observed in the crib on 2/12/2020 while the crib was not occupied.
2. Child # 4 was observed sleeping in a crib on 2/12/2020 with two soft pillows along both sided of the baby's body.

Description: Based on observation, an occupied infant bouncy seat was occupied and no safety strap was in use.
Evidence:
An infant was in the seat upon arrival and the Inspector did not see the safety strap in use.

Description: Based on observation, a child was left unattended on the changing table during diapering.
Evidence:
During the inspection, the Licensing Inspector observed the provider step away from the changing table and leave a child unattended while the child was laying on the table for several seconds. The provider had her back to the table when she was talking to another child.

Description: Based on provider statement and observation, a soiled diaper was not disposed of in a foot operated trashcan.
Evidence:
The provider was observed disposing of a diaper during the inspection into the trashcan outside the back door. When questioned by the Inspector, she explained the soiled diaper had been left in the sink.
