Technical Assistance:
Licensing inspector had conversations with the provider on the following topics:
- hand washing
- appropriate clothing and accessories for outdoor play; the children were observed playing outside without shoes, socks or coats.
- offering structured activities for the children.
- an evacuation drill has not yet been practiced for the month of February; to day is the last day of the month.
- if a parent requests that another biological parent not be permitted to pick up a child, court ordered documentation must be obtained.

Comments:
An unannounced monitoring/renewal inspection was conducted today between 11:10am and 1:35pm. There were 12 children present with the Provider and 1 assistant for a total of 27 points within the supervision guidelines. The children were observed playing freely indoors, outdoors and napping. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-900-B
Description: Based on observation, children were allowed to drink while lying down.
Evidence: Child #9 was observed drinking a bottle of milk while laying in their crib.

Plan of Correction: I'll start feeding him outside of his crib.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider has not obtained documentation of a physical exam prior to a child's attendance or within 30 days after their first day of attendance.
Evidence: Child #10 (date of enrollment: 8/6/2018) and Child #11 (date of enrollment: 3/5/2018) files do not contain documentation of a complete physical exam.

Plan of Correction: I'll tell her today that I need the physicals.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child #2 file does not contain the families medical insurance information.
1. Child #1, Child #2, Child #10 and Child #12 files do not contain documentation of an annual review of emergency contact information.

Plan of Correction: I'll have the parents do it today or Monday.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider has not obtained a subsequent tuberculosis (TB) screening every two years from the date of the first screening for each caregiver.
Evidence:
1. Caregiver #1 most recent documented TB screening is dated 1/15/2017.
2. Caregiver #2 most recent documented TB screening is dated 1/12/2017.

Plan of Correction: We did it but I can't find the results. I'll contact the doctors office to see if they can't the results again for us.

Standard #: 22VAC40-111-250-A
Description: Based on observation, ropes were not out of reach of children under the age of five.
Evidence: The provider currently has two ropes suspended from a closet shelf, wrapping around a gate that holds toys into a closet. The children were observed using the rope to pull themselves up over the gate and playing in the closet full of toys.

Plan of Correction: I'll remove the ropes today.
