A monitoring inspection was conducted today. There were 8 (22 points) children present with 3 caregivers present. The physical plant, children's records, caregiver records, household member records, the outside play area, cribs, bottles, first aid kit, menu's and the emergency preparedness plans were reviewed. Standard #: 8VAC20-800-760-A Description: Based on observation, the provider failed to ensure that all required supplies were contained within the first aid kit.
Evidence:
The first aid kit did not contain an instructional manual.Plan of Correction: I will purchase a first aid manual for the first aid kit.
Standard #: 8VAC20-800-920 Description: Based on observation, the provider failed to have a current menu posted or placed in an areas accessible to parents. Plan of Correction: I will print and post the current menu/
Standard #: 8VAC20-800-60-A Description: Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.
Evidence:
Child #1's record did not contain documentation of liability insurance form, medication policy or emergency plan.
Child #3's record did not contain documentation of a second emergency contact, liability insurance form or the emergency plan.
Child #5's record did not contain the phone number for the physician.Plan of Correction: I will get the information from the parents and place in children's files.
Standard #: 8VAC20-800-690-D Description: Based on observation the provider did not ensure that the diapering surface will be nonabsorbent and washable.
Evidence:
The changing pad had holes in it and therefore was not nonabsorbent.Plan of Correction: I will purchase a new changing pad for the changing table.
