Based on record review and staff interview, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of suspected or confirmed allergic reaction.
Evidence: Child #6 and Child #7 both have diagnosed food allergies and neither child had a written allergy care plan on file signed by a doctor.

We have filled the appropriate forms out and are just waiting on doctor's signature.

Based on record review, the center did not ensure that all staff complete the Virginia Department of Education sponsored orientation course within 90 calendar days of employment.
Evidence: Staff #1 (Date of Hire: 9/1996) did not have documentation on file of having completed the Virginia Department of Education sponsored orientation course.

This course will be completed.
