Based on observation and interview, the center did not ensure when a group of children receiving care included children from different age brackets, the age of the youngest child in the group was used to determine the staff-to-children ratio that applies to that group.
Evidence: 1. At approximately 10:00am, the licensing inspector observed 17 children with two staff members in one room. The children?s ages ranged from two years to four years of age. 2. Approximately five minutes later, eight children and one staff member went to the Two?s classroom. Nine children remained in the classroom with one staff member. Child #1 remained with the nine children. Child #1 was two years of age. The other eight children were three years or older. 3. A member of management confirmed Child #1 was two years old. 4. The required staff-to -child ratio is one staff member for every eight children present.

A member of management confirmed this information while the inspector was on site. 4. The parent handbook states a copy of immunization records is required to be obtained prior to attending the center.
