Technical Assistance:
In order to make the license and most recent violation notice more readily available to current participants and their families please post a copy of both documents near the participant entrance.

Comments:
An unannounced mandated monitoring inspection was conducted on 6/6/16. At the time of entrance 32 participants were present with 7 center staff including the Center Nurse and Assistant Director. This far exceeds the required ratio of one staff per six participants. The sample size consisted of four participant records, three staff records and two individual interviews. Participant and staff records and other documentation reviewed. Criminal Background Checks of all staff hired since previous inspection conducted on 5/12/15 were reviewed. Participants were observed having a morning snack and engaging in activities including trivia, name that tune, walking club and morning exercise. Medication cart inspected. No participants received medications during the inspection. No violations cited and exit interview held.
