Technical Assistance:
Please note that staff are now required to complete the Sworn Disclosure form yearly and are required to inform the facility Administrator if convicted of a barrier crime.

Comments:
An unannounced renewal inspection was conducted on 2/13/17. At the time of entrance 37 participants were present with 10 center staff including the Assistant Director. This exceeds the required ratio of one staff per six participants. The sample size consisted of six participant records and 12 staff records including the criminal background checks on all new staff hired since previous inspection. Spoke with participants. and participants were observed eating and engaging in activities including live music and dance, and morning exercise. Medication administration was reviewed .
