Comments:
An unannounced renewal inspection was conducted on 7-16-18 (ar 07:55 am/dep 18:00 pm) and 7-19-18 (ar 09:10 am/dep 12:00 pm). The facility census on 7-16-18 was 32 residents. A tour of the facility conducted with administrator, water temperatures, emergency supply checked, staff and resident records reviewed, first aid kit for facility and vehicle checked, staff, resident and family interviews conducted. A small activity was observed, resident council minutes reviewed and medication cart checked. Comments: LI suggested facility review and revise medication management plan to comply with what was observed and what is being conducted in the facility. Medication dosing hour has a 2-hour range, unable to determine compliance for 1 hour before or 1 hour after per regulation. Discussed with administrator, director of nursing and resident care coordinator compliance concern and developing policy and procedures to submit to the department for approval of current practice. A review of current standards was discussed as necessary for clarification and compliance. Facility reminded to follow assisted living guidelines for all staff hiring and training requirements, particularly employees that are shared with the adjourning nursing center, particularly as it relates to date of hire and criminal record requirement per assisted living requirements. Also discussed was TB requirement for signature compliance also discussed when document is completed by facility staff/ letter from physician requirement. The exit interview conducted with the administrator, director of nurse and nurse staff on both days. The acknowledgement form was completed with the administrator on both days of the inspection.
