An unannounced non-mandated monitoring inspection was conducted on 7/24/18 from approximately 1:30 pm to 4:30 pm regarding an incident of a resident exiting the facility. Interviews were conducted with residents, family members, staff and outside agency staff. Resident records were also reviewed. There were no violations as a result of this inspection.
