Comments:

Licensing Inspector (LI) conducted an unannounced complaint investigation on 5/24/19 regarding resident care and dining procedures and staff qualifications and actions during an incident that occurred on 2/14/2019 and was reported by the facility on 2/15/2019. LI reviewed resident record including Uniform Assessment Instruments, Individualized Service Plans, Medication Administration Records, and physician and dietary assessments, orders, and progress notes. Staff records reviewed including licenses, certifications and training. Resident Emergencies Policies and Procedures reviewed. Interviews conducted. Spoke with Executive Director and Director of Nursing. Complaint regarding resident care and dining procedures and staff qualifications and actions during an incident that occurred on 2/14/2019 is deemed not valid as a preponderance of evidence gathered during the investigation did not support the allegations.
