An unannounced complaint inspection was conducted by the licensing inspector on June 3, 2019 from 12:45 p.m. to 1:45 p.m. The complaint was regarding concerns of physical neglect of a resident. A census of 8 residents was reported. Upon interviewing the resident, staff, and reviewing facility documentation, as well as the resident's record, the complaint was determined to be not valid.
