Comments:
An unannounced inspection was conducted by two Licensing Representatives from the Eastern Regional Office on May 23, 2019 from 10:15 a.m. to 11:50 a.m. There were 77 residents in care. During the inspection, resident and private duty personnel records were reviewed, along with video footage in the facility due to a concern for a resident having an uninvited visitor. The following was discussed during the inspection: Ensuring that the most current violation notice is posted in the facility visible to the public, education on standards related to private duty personnel, the facility?s policies to address visitors, and training for staff. Best practice shared regarding initiation of possible bag policy implementation to ensure safety. Please submit your ?plan of correction? and ?date to be corrected? in a Word document for each violation cited on the violation notice and return it within 10 calendar days. The plan of correct must indicate how the violation will be or has been corrected. Just writing the word ?corrected? is not acceptable. The plan of correction must include: 1. Step(s) to correct the noncompliance with the standard(s) 2. Measures to prevent reoccurrence 3. Person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on record review and interview, the facility failed to ensure that when private duty personnel from licensed home care organizations provide direct care or companion services to residents in an assisted living facility, that the facility shall provide orientation and training to private duty personnel regarding the facility?s policies and procedures related to the duties of private duty personnel. Evidence: 1. On 05-23-2019 during review of Resident #1?s private duty aide?s record, it was discovered that Private Duty Aide #1 was not provided orientation or training to the facility, but only to the agency that she was hired through. 2. Video footage reviewed on 05-23-2019 revealed that Private Duty Aide #1 had brought an unknown visitor to Resident #1?s room on 05-14-2019, which showed the visitor entering at 7:34 p.m. and exiting at 8:11 p.m. 3. Staff #1 and Staff #2 confirmed that the facility did not provide an orientation or training to Private Duty Aide #1 that involved facility specific policies and procedures, and that Private Duty Aide #1 had brought an unwelcomed visitor into Resident #1?s room on 05-14-2019.

Plan of Correction: Private duty caregiver orientation was provided by the private duty organization. Steps to correct the noncompliance with the standard: All private duty caregivers will be provided orientation to Mennowood Retirement Communities policies and procedures related to duties of private duty personnel by a Mennowood employee. Measure to prevent noncompliance: Administrator or designee will audit private duty caregiver files monthly to ensure that they are provided orientation.
