If you need a copy of any of the DSS Model forms or to review any inspection or regulation, you can find the information on the internet: www.dss.virginia.gov. Inspection 8:30am-noon. Census 79 Number of records reviewed and interviews conducted- 2 records, 8 interviews. All facility self-reported incidents since the last inspection were reviewed on this date. There was an allegation of staff failing to administer medications as required. The complaint was determined to be valid. There were violations found during the inspection. The Licensing Inspector and the Administrator discussed the risk assessment ratings for the violations for this inspection. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to the office. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on resident record review and staff interview, it was determined that the facility staff failed to document the administration of physician ordered medications as required. Evidence: Resident A had no documentation of the administration of the following physician ordered medications for June 2, 2019: 8am Amlodipine Besylate 10mg, Aspirin 81mg, Centrum Silver, Hydrooxyzine HCI 25mg, Paricalcitol 1mcg, Vitamin D3, and Cranberry capsule 500mg. Resident B had no documentation of the administration of the following physician ordered medications for June 2, 2019: Amlodipine Besylate 10mg, Certavite 18mg, Fluticasone spry 50mcg, Lasix 20mg, Namenda 28mg, Polyethylene Glycol, Vitamin C 500mg, Calcium with vitamin D 600mg, Check blood pressure, Depakote 125mg, Loratadine 10mg, Sarna Lotion, and Dorsolamide Timolol 2% drops.

Plan of Correction: All medications will be administered as ordered by the physician. The MARs will be audited to ensure compliance with the physician orders.

Description: Based on resident record review and staff interview, it was determined that the facility failed to have documentation in a record of physician ordered procedures or treatments. Evidence: Resident B had no documentation of the applying of the compression stockings in the morning of June 2, 2019. There was no documentation of the removal of the compression stockings in the evening of June 10 and 24, 2019.

Plan of Correction: All physician treatments and procedures will have documentation within the resident record as required.
