Comments:
An unannounced complaint inspection was conducted by the Licensing Inspector from the Eastern Regional Office on 07-16-2019 from 1:05 PM to 2:25 PM. There were 46 residents in care at the time of the inspection. The complaint was regarding Staffing and Supervision and Resident Care and Related Services. Two resident records and staff schedules were reviewed. Interviews were also conducted. Based on the findings during this inspection, the complaint was found not valid; however, other areas of noncompliance were found. The facility received a violation "under" Administration and Administrative Services. LI reviewed the areas of noncompliance with the staff person in charge/RMA due to the Administrator not being present during the inspection. Please complete the -Plan of Correction- for each violation cited on the violation notice and return it to me within 10 calendar days from today, on 08-01-2019. You will need to specify how the violation will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must include: 1. steps to correct the noncompliance 2. measures to prevent reoccurrences 3. person(s) responsible for implementing and monitoring each step of the corrective measures and / or the preventative measure.

Description: Based on record review and interview, the facility failed to report to the regional licensing office within 24 hours any major incident that has negatively affected or that threatens the life, health, safety, or welfare of any resident. 1. On 07-15-2019, the facility faxed an incident report to the regional licensing office regarding a resident to resident altercation involving resident #1 and resident #2. The report documented "resident #2 assaulted resident #1 while he was in his room laying in bed." The date and time of the incident was 07-13-2019 at 7:17 AM; however, the incident was not reported to the regional licensing office within 24 hours. 2. During resident #1?s record review with staff #1, a hospital summary dated 07-13-2019 documented the reason for visit was "Reported Assault Victim." The summary also documented the resident had a diagnosis of lip laceration, facial laceration, facial contusion, alleged assault, and dental injury. 3. During interview, staff #1 acknowledged the aforementioned incident was not reported to the regional licensing office within 24 hours.

Plan of Correction: Hill's Home Administrator and Office Manager had a meeting and has agreed that they will be responsible for turning in any incidents that occur at Hill's Home for Adult within 24 hours to their state inspector. Hill's Home Administrator and Office Manager has agreed to turn in any incidents that occur at Hill's Home for Adults within 23 hours instead of waiting to do it within 24 hours. Hill's Home Management and Administrator will make sure that Staff at Hill's Home for Adult will continue to fill out the incident report and report all incidents to management as they occur. Hill's Home for Adult Management will continue training for their staff in regards to reporting incidents and on Crisis Intervention, Mental Illness, and Behavior Training and more.
