Standard #:	22VAC40-73-1050-A
Description:	Based on chart review and interviews, it was determined the facility did not provide direct care staff supervision while residents with serious cognitive impairments are outside. Evidence: Resident A had a physician assessment upon admission in December of 2018, that the resident had a serious cognitive deficit. The resident had been allowed to go outside without direct care supervision.
Plan of Correction:	Review charts and update care plan to reflect resident need to have supervision while outside
