Comments:

An unannounced renewal inspection was conducted by two licensing staff on August 12, 2019 from 10:30 a.m. to 12:15 p.m. A follow-up visit was conducted by the licensing inspector on August 14, 2019 from 7:40 a.m. to 8:40 a.m. A census of 8 residents was reported. Previous violations were reviewed and corrected. The following items were reviewed/observed during the inspection: sample of 4 resident records and 3 staff records, tour of the facility, facility documentation, menu/activity schedule, first aid kit and emergency food/water supplies, medication pass observation/physician's orders, Medication Administration Records (MARs), and resident/staff interviews.
