Comments:
An unannounced monitoring inspection was conducted on 8/7/19, to follow-up on a high-risk violation that was cited on 6/17/19. Medication administration was observed and resident records were reviewed. Violations were discussed and an exit meeting was held. Areas of non-compliance are identified on the violation notice. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return to the licensing office within 10 calendar days. Please specify how the deficient practice will be or has been corrected. Just writing the word 'corrected' is not acceptable. The 'plan of correction' must contain: 1) Steps to correct the non-compliance with the standards, 2) Measures to prevent the non-compliance from occurring again, and 3) Person responsible for implementing each step and/or monitoring any preventative measures. Description: Based on documentation and interview, the facility failed to report to the regional licensing office within 24 hours any major incident that threatens the life, health, safety, or welfare of any resident. Evidence: The facility contacted the Virginia Department of Health on 7/12/19, due to concerns about several residents with respiratory illness symptoms. The licensing office was not contacted about the symptoms that the residents were experiencing.

Standard #: 22VAC40-73-680-D
Description: Based on observation and record review, the facility failed to ensure that medications are administered in accordance with the physician's instructions. Evidence: The morning medication administration, for Resident #1, was observed during the inspection. Resident #1's record contained an order, dated 1/30/19, that calls for the resident to receive Omeprazole once daily before eating. During the inspection, Resident #1 received Omeprazole and the rest of his morning medications. After his medication was administered, Resident #1 reported that he had already eaten.
