Comments:
An unannounced monitoring inspection was conducted by two licensing representatives on September 11, 2019 from 11:20 a.m. to 1:00 p.m. A census of 29 residents was reported. There were no previous violations. The following items were reviewed/observed during the inspection: sample of 6 resident and 3 staff records, facility documentation, tour of the facility, medication pass observation/physician's orders/Medication Administration Records (MARs), resident/staff interviews, lunch meal/menu/activity schedule, emergency food and water, and first aid kit supplies. Violations cited are identified in this report. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 10 calendar days. Please specify how the violation will be corrected. The plan must contain: 1) step(s) to correct the non-compliance with the standard(s), 2) measures to prevent the non-compliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: 22VAC40-73-210-B
Description: Based on the review of staff records, the facility failed to ensure that all direct care staff attended at least 18 hours of training annually.

Evidence:
-Record for Staff # 1 did not contain documentation of annual training. Assistant Administrator stated that staff records had recently been purged and training for Staff # 1 could not be found.
-Record for Staff # 3 contained documentation of only 3 hrs. of annual training.

Plan of Correction: Assistant Administrator will ensure that all direct care staff attend at least 18 hours of training annually. Staff # 1 and # 3 will be scheduled for in-service training. Documentation of training will be kept in staff record.

Standard #: 22VAC40-73-210-F
Description: Based on a review of staff records, the facility failed to ensure that at least two of the required hours of staff training focus on infection control and prevention.

Evidence:
-Record for Staff # 1 and Staff # 3 did not contain documentation of 2 hours of annual infection control training.

Plan of Correction: Assistant Administrator will ensure that all staff complete two hours of infection control and prevention training and document in staff record. Staff # 1 and # 3 will be scheduled for training.
