Comments:

Two licensing inspectors conducted an unannounced focused monitoring inspection regarding a self reported incident at Commonwealth Senior Living-Christiansburg on 09/03/2019. The inspection started at 10:15 am and concluded at 12:16 pm. A tour of the building was conducted, video surveillance was observed, and Medication Administration Records (MARs )and staff files were reviewed. As a result of this inspection, no violations are being cited. An exit meeting was conducted with the administrator of the facility on 09/03/2019 and at that time the opportunity was given to find items that could not be located in files. Please return a signed and dated copy of the violation notice back to your licensing inspector within 10 calendar days (10/31/2019) of receipt.
