An unannounced focused inspection was conducted on 10/23/19 by 2 licensing inspectors from approximately 11:10 a.m. - 12:30 p.m., to follow up on areas of non-compliance resulting in a high risk violation. During the inspection, 1 resident record was reviewed and facility documentation was reviewed. The issue reported in the complaint, which lead to violation with high risk has been resolved and the resident noted in the complaint is no longer residing at the facility.
