Comments:
Two licensing inspectors conducted an unannounced license renewal inspection at Commonwealth Senior Living in Abingdon, on 10/30/2019. The inspection began at 9:45 am and concluded at 1:10 pm. A tour of the building and grounds was conducted. Residents and collateral were interviewed. Resident and staff interactions were observed. The noon meal, activities,and the morning medication pass were observed. Resident and staff files were reviewed. Medications and MARs (Medication Administration Records) were observed and medication carts were audited. The facility is licensed to provide care to 74 residents, and 65 residents were found to be in care at the time of the inspection. Required postings and the previous inspection was observed to be in place. An exit meeting was conducted with the administrator and other key staff on 10/30/2019 and at that time the opportunity was given to find items that were not readily available in the records. As a result of this inspection, two violations are being cited. A corrective action plan should be developed addressing steps to correct the non-compliance of each standard; measures to prevent the re-occurrence; and person(s) responsible for implementing each step and/or monitoring and prevention measures. The "description of action to be taken" for each violation along with the "date to be corrected" must be returned to this office signed and dated within 10 calendar days (11/09/2019) of receipt. Standard #: 22VAC40-73-870-A
Description: Based on observations made during the morning tour of the building, the facility failed to ensure all areas of the interior of the building were kept clean and free of rubbish.

EVIDENCE:
1. The licensing inspector observed the floor in the dining room area on the assisted living side to be dirty. There were several spots on the floor throughout the dining room where drinks had been spilled. There was also food crumbs found underneath the dining room tables .

Plan of Correction: Dining Room was immediately cleaned. Cleaning schedule was implemented and associates were educated on the cleaning schedule and assignments to ensure routine and daily cleaning. Dining Services Director will ensure compliance with the cleaning schedule. Executive Director will complete random cleanliness audits a minimum of once per day to ensure continued compliance. [sic]

Description: Based on observations made during the morning tour of the building, the facility failed to ensure all interior lighting was adequate for the safety and comfort of residents and staff.

EVIDENCE:
1. The licensing inspector observed a bedside lamp flickering in resident room number 403 in the memory care unit. The lamp was located on the side closest to the window.

Plan of Correction: Light bulb was replaced in the lamp in room 403. Remaining areas were inspected to ensure that there were no additional concerns. Direct care associates and housekeeping associates re-inserviced on completing maintenance repair requests when items are in need of repair. Maintenance Director will continue to round in the community daily and Executive Director will round in the community a minimum of 2 times per day to ensure continued compliance. [sic]
