Comments:
An unannounced complaint inspection was conducted by the Licensing Inspector from the Eastern Regional Office on 10-09-2019 from 11:52 AM to 12:50 PM. The complaint alleged concerns with Buildings and Grounds.There were 71 residents in care at the time of the inspection. A tour of the facility and interviews were conducted. Lunch was observed. Based on the results of this inspection, the complaint is not considered valid at this time. The facility received violations that were not complaint related "under" Resident Accommodations and Related Provisions and Buildings and Grounds. The areas of noncompliance were discussed with the Administrator throughout the inspection and during the exit interview. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the Violation Notice and return it to me within 10 calendar days from today on 10-21-2019.

Description: Based on observation and interview, the facility failed to ensure the residents always have clean bed linens to include pillowcases and sheets.
Evidence:
1. During the tour of the facility with staff #1, staff #2, and staff #3, the following bed linens observed were not clean:
A. In room #19, approximately 7 gnats flew away when the covers were pulled off of bed #1.
B. In room #22, there was an oval shaped yellow stain on the fitted sheet of bed #1, and there were multiple pin point sized specks of a black substance located on the fitted sheet of bed #2.
C. In room #5, there were three dime sized orange stains and specks of a black substance located on the fitted sheet of bed #2.
D. In room #2, the fitted sheet was missing on bed #2.
E. In room #46, there were brown stains throughout the pillowcase on bed #1.
2. During interview, staff #1, staff #2, and staff #3 acknowledged the aforementioned bed linens were not clean.

Plan of Correction: A. All resident beds are changed 1xwk and PRN according to care plan. Many of our clients go out and bring fresh fruits etc. from day programs or stores and hide it, creating gnats. Orange stains located on sheets was pointed out to inspector during inspection that resident was eating cheese crackers in bed, with evidence of a package of cheese/pnut butter crackers at their bedside. Specks found on the sheets were loose pieces of tabacco, Resident with stains on pillow case was from hair oil, that he uses several times daily, Resident also takes naps during the day. Resident refuses to wear hair cap when taking naps or at bedtime.
Staff will continue to make rounds and ensure that all beds are made with proper linen. Staff is encouraged if they have to change beds to make bed as soon as possible.

Description: Based on observation and interview, the facility failed to ensure all showers are kept clean and in good repair.
Evidence:
1. During the tour of the facility with staff #1, the shower drain was clogged with standing water, and standing water was observed throughout the floor of the men?s bathroom located near room #17.
2. During interview, staff #1 acknowledged the shower drain was not in good repair and the floor of the men?s bathroom was not clean.

Plan of Correction: Bathtub with shower in mens shower room drains okay shower. Free standing shower in men's bathroom is temporarily down. Bathtub with shower is being replaced with 2 free standing showers.
Housekeeping and nurses will monitor shower rooms during each shift to ensure that all drains are draining properly.
