Comments:
Unannounced inspections were conducted on 6/26/19, 7/20/19, and 12/10/19. Interviews were conducted and the resident record was reviewed. Additional information was received after the facility visits. The violation was discussed and an exit meeting was held. Areas of non-compliance are identified on the violation notice. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return to the licensing office within 10 calendar days. Please specify how the deficient practice will be or has been corrected. Just writing the word 'corrected' is not acceptable. The 'plan of correction' must contain: 1) Steps to correct the non-compliance with the standards, 2) Measures to prevent the non-compliance from occurring again, and 3) Person responsible for implementing each step and/or monitoring any preventative measures. Violations:
Standard #: 22VAC40-73-1130-D
Description: Based on documentation and interviews, the facility failed to ensure that there were sufficient direct care staff to provide sight and sound supervision to residents, during trips away from the facility.
Evidence: Resident #1 went on a facility outing with several other residents on 6/20/19. Facility staff reported that three staff members attended the outing. Toward the end of the outing, Resident #1 wandered away from the group and staff supervision. Facility staff discovered that Resident #1 was missing, during a resident head count. An area search was conducted and 911 was called.

Resident #1 was located approximately a half-mile away from the outing location and an ambulance transported her back to the group. The record for Resident #1 includes an assessment of serious cognitive impairment, dated 1/31/19. The assessment states that Resident #1 has a serious cognitive impairment and that the resident is unable to recognize danger or protect her own safety and welfare.

Plan of Correction: During outings the Programming Director will ensure that there is a staff member for every 3 residents that attend an outing where the residents depart the bus. Any outings that the residents remain on the bus the entire time will require at least 2 staff members for 10 residents. The Executive Director will review the outing list prior to outings to ensure staffing ratio is followed.
