Comments:
Two licensing inspectors conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 01/24/2020. Building observations were completed and interviews were conducted with residents and staff and resident records were reviewed relating to allegations building issues, staffing issues, the lack of physician's orders, and the lack of activities. The preponderance of the evidence gathered during the investigation on 02/06/2020, and 02/21/2020 does support the allegations, so the complaint will be determined to be valid. Two violations are being cited as a result of this complaint inspection. On 02/06/2020 the complaint inspection began at 10:17 am and concluded at 11:28 am. The inspection began at 10:00 am and concluded at 10:55 am on 02/21/2020. An exit meeting was held with the designated person in charge on 02/06/2020 and 02/21/2020 and at both of those times, the findings of the investigation were shared. Please develop a plan of correction for each violation being cited and retun a signed and dated copy of the violation notice back to the licensing office within 10 calendar days of receipt (03/09/2020). Description: Based on the tour of the memory care section of this community, the facility failed to ensure the interior of all buildings were kept clean and in good repair and free from rubbish.
EVIDENCE:
1. At approximately 10:30 am two licensing inspectors entered the memory care section of this community. There were at least three sections of the dining room in which breakfast particles were noticed on the floor. Staff # 1 stated breakfast was served at 8:00 am in the memory care section of the community.
2. Licensing Inspectors observed the floor to be sticky and tacky as if it had not been mopped.

Plan of Correction: Area was cleaned during the inspection. Staff re-educated on cleaning schedule. Maintenance Director re-educated on physical plant reviews to ensure community is maintained in accordance with Licensing standards. ED will continue to round in the community daily with random focused physical plant monitoring to ensure continued compliance. [sic]

Description: Based on observations made during the tour of the memory care of the community, the facility failed to ensure all building shall be well-ventilated and free from foul, stale, and musty odors.
EVIDENCE:
1. When two Licensing Inspectors entered the memory care section of the community, a smell of urine was noticeable.
2. Staff # 1 stated there was a resident in the memory care section of the community that did urinate in the floor especially along the entrance/exit to the memory care where decorative trees are used to create the look of the outdoors. Staff #1 stated this resident was accustomed to urinating outside prior to residing in this assisted living facility as she thought he lived in a camper and spent a lot of his time in the outdoors.

Plan of Correction: Removed the source of the urine odor in memory care neighborhood. Re-inserviced direct care associates to bag soiled undergarments immediately upon removal and place in disposal are to ensure continued compliance. More frequent toileting is provided for residents with continence issues. Maintenance Director will continue to round community daily and Executive Director will round in the community a minimum of 2 times per day to ensure continued compliance. [sic]
