Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on 05-20-2020 and concluded on 05-21-2020. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census was 21. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed 3 resident records, 3 staff records, criminal background checks and sworn disclosures of newly hired staff, staff schedules, fire drills, fire and health inspection reports, and healthcare oversight. Information gathered during the inspection determined non-compliances with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Based on record review and interview, the facility failed to ensure the healthcare oversight was provided at least every three months for residents who meet the criteria for assisted living care. Evidence: 1. Resident #3?s Uniform Assessment Instrument dated 12-01-2019 documented the resident was assessed at the Assisted Living level of care. 2. Staff #1 stated the facility's healthcare oversight is provided by a licensed healthcare professional from a home health agency. The licensed health care professional is not employed by the facility on a full-time basis. 3. Staff #1 provided a copy of the facility?s healthcare oversights. The healthcare oversights were not provided at least every three months: A. The most current healthcare oversight was provided on 04-17-2020, with a review date of October 2019 through March 2020. B. The previous healthcare oversights were provided on 10-07-2019, with a review date of April 2019 through September 2019; and on 04-06-2019, with a review date of November 2018 through March 2019. 4. Staff #1 acknowledged the health care oversights were not provided at least every three months.
Plan of Correction:	The facility will ensure that the healthcare oversight is done at least every three months for residents who meet the criteria for assisted living care.
