This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring)inspection was initiated on 05/15/2020 and concluded on 05/15/2020. A self-reported incident was received by the department regarding allegations in the areas of Resident Care and Related Services. The Administrator was contacted by telephone to conduct the investigation. The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-73-450-C
Description: Based on record review the facility failed to ensure the comprehensive individualized service plan included a written description of what services will be provided to address identified needs.

Evidence: On 05/15/2020 while reviewing the individualized service plan for resident #1, the need for cognitive orientation reads, " Resident is disoriented to some spheres, all of the time. Resident is disoriented to place, time and situation. Staff will redirect." The individualized service plan, does not address how staff will redirect resident #1 to address the identified need.

Description: Based on resident record review the facility failed to provide supervision of resident schedules including attention to specialized needs, such as wandering from the premises.

Evidence#1: On 05/15/2020 while reviewing resident #1's record, the facility nurse note dated 03/18/2020, 8:00am reads, " While staff was making rounds, the resident was not in the apartment. All staff searched the building and the resident could not be located." "All available staff searched the grounds outside of the building and the resident was discovered at the bottom of the hill behind the maintenance shed." Based on statement provided by staff #1, the resident had been seen 10-15 minuets after 8:00am by hotel staff located near the facility after the search begin. Resident #1 was transported to the Emergency room and seen for fall, initial encounter, dementia and abrasions.
