Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on 5-27-20 and concluded on 6-11-20/ A complaint was received by the department regarding allegations in the areas of staff general qualifications.
The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violation was issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Complaint related: Yes
Description: Based on interview, the facility failed to ensure staff was considerate and respectful of the rights, dignity, and sensitivities of a person who is aged, infirm, or disable.

Evidence:
1. During the remote complaint inspection, resident #1 was alleged to have not been spoken to and treated with respect on the afternoon of 4-23-20. During interviews, it was reported, staff #1, stood and spoke in an aggressive tone and pointed finger at the resident #1.
2. A review of resident #1's record revealed resident diagnosed with a brain injury and requires redirection, because of forgetfulness.

Plan of Correction: 1. Staff #1 completed computer based in-service on Challenging Behaviors.
2. An immediate in-service was initiated for all departments on Challenging Behaviors.
3. Challenging Behaviors will be added to the orientation agenda by the educator/designee as well as assigned semi- annually as a refresher course. All residents will continue to be treated with respect and dignity. Facility leadership and the facility grievance officer will be re-educated on the Grievance Policy by the educator/designee. For resident grievances, a meeting will be offered with multiple directors in order to hear their voiced concerns.
4. The educator/designee will conduct two staff interviews weekly for eight weeks on how to manage challenging behaviors and/or the grievance process. Results of these interviews will be reviewed by the QA Committee for continued improvement and analysis.
