Comments:
This inspection was conducted by licensing staff using an alternate remote protocol
necessary due to a state of emergency health pandemic declared by the Governor of Virginia.A monitoring inspection was initiated on 06/24/2020 and concluded on 06/24/2020. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed 4 resident records, 4 staff records and additional documents submitted by the facility to ensure documentation was complete. Violations:
Standard #: 22VAC40-73-650-A
Description: Based on staff interview and resident record review, the facility failed to ensure no medication, dietary supplement, diet, medical procedure, or treatment shall be started, changed, or discontinued by the facility without a valid order from a physician or other prescriber.

Evidence #1: While reviewing resident #4's medication administration record for May, 2020 on 06/24/2020 with staff #1, #2 and #3, resident #4 physician order sheet with effective date of 5/16/2020 orders, " Levothyroxine Sodium 100 mcg tab administer one tab by mouth before breakfast 6am". While reviewing the medication notes on 06/24/2020 for resident #4, the medication Levothyroxine Sodium 100mcg is written under the reason given/held as the following: "5/03 6am, med is on the cart is wrong, 5/4, 6am med listed as 112 mcg but 100mcg was given. 5/9, 6pm, outside of parameters, 5/10pm, outside of parameters, 5/11 6pm outside of parameters, 5/12 6pm, outside of parameters, 5/13pm medication is documented wrong time, 5/14/2020 med was given in the morning, 5/15, 6pm duplicate." Based on statements of staff #1, #2 and #3 the physicians order was changed effective 04/30/2020 and the facility administered resident #4 112mcg ,"as this was all that the facility had". The facility was not able to provide the Licensing Inspector a copy of resident #4's physician orders for Levothyroxine Sodium 112mcg.

Standard #: 22VAC40-73-680-D
Description: Based on resident record review the facility failed to ensure medication shall be administered in accordance with the physician's or other prescribers instructions and consistent with the standards of practice outlined in the current registered medication aide curriculum approved by the Virginia Board of Nursing.

Evidence #1: On 06/24/2020 while reviewing resident #4's physician order sheet with the effective date of 05/22/2020 the order reads, "Warafin sodium 2mg tabs administer 2mg every Sunday, Tuesday, Wednesday, Thursday, Friday and Saturday". The medication administration record for the month of May, 2020 evidences that no medication was administered for the 6, 8, 10, 11, 13, 15 and 18. Based on interview with staff #1, #2 and #3 the facility had no additional documentation to provide.

Evidence #2: On 06/24/2020 while reviewing the May medication administration record for resident #3, the physician order sheet for Clonazepam dated 05/06/2020 reads, " Clonazepam 0.5mg tablet ( KlonoPIN) take 0.5tab (0.25mg) by mouth at bedtime-8pm Indicated for anxiety). Resident #3's medication administration record for Clonazepam" Take 0.5 tab (0.25mg) by mouth at bedtime is circled and initialed by staff #4. Resident #3's medications notes entered by staff #4 reads, " 5-6 clonazepam 0.5 tablet other: This is bed time med". Resident #3's medication administration record for the same medication with administration time of 8:00pm on 05/06/2020 reflects no medications was administered. Based on statements provided by staff #1, #2 and #3 the facility had no additional documentation to provide.
