A Licensing Inspector with the Division of Licensing conducted an announced, non-mandated, initial inspection on 12/18/2019 from approximately 11:10 am to approximately 12:05 pm. At the point of entrance the facility Administrator was available and on-site with 70 residents in care. During the facility inspection the Licensing Inspector observed the facility assisted living and memory care unit to include resident rooms, tested the facility call bell system and the facility water temperatures, observed the facility emergency food and water supply, observed all required items posted and reviewed the facility first aid kit. During the facility inspection the Licensing Inspector observed residents engaged in activities on the facility memory care unit and assisted living. Areas of non-compliance are found within this violation notice. Please complete the "plan or correction" and "date to be corrected" for each violation cited on the violation notice and return to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring preventative. Standard #: 22VAC40-73-930-A Description: Based on a test of the facility call bell system the facility failed to ensure all assisted living facilities have a signaling device that alerts direct care staff that a resident needs assistance.
Evidence: On 12/18/2019 with the facility Administrator, while testing the facility call bell system in resident room #110 and #114 it was observed that the call bell system did not alert the direct care staff. Based on the Administrator statement the call bell system for resident room #110 and #114 battery was low.Plan of Correction: Resident room #110 and #114 was repaired by maintenance on 12/18/2019 while on site and is now functioning properly. A 100% audit of all resident rooms and common area call bell systems was completed on 12/18/2019 to ensure they are functioning properly. ED/nursing staff will be re-educated to call system and how to test for proper function.
