Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on 09/04/2020 and concluded on 09/08/2020. A complaint was received by the department regarding allegations in the area of resident care and related services. The Administrator was contacted by telephone to conduct the investigation. The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description:	Based on documentation review, the facility failed to ensure that the complete resident record was retained for at least two years after the resident leaves the facility and was made available at the facility within 48 hours. EVIDENCE: 1. Upon requesting a copy of the record for resident 1, Collateral 1 received a letter, dated 07/27/2020, from the facility?s legal representation which included the statement ?that the entire chart has been misplaced or lost?. 2. Interview with Collateral 2, reports that as of 09/04/2020 the full record for resident 1 had still not been received.
Plan of Correction:	Facility will maintain complete resident records onsite for one year and retained off in safe secure area and be available at facility within 48 hours.
