This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
The evidence gathered during the monitoring inspection noted non-compliance standards or law in the area or personal care and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Standard #: 22VAC40-73-460-B
Description: Based on record review and staff interview, the facility failed to ensure prompt response by staff to a resident's need as reasonable to the circumstances.

1. On 8-11-20 during interview with resident #1, the inspector was informed, resident asked staff for pain medication but request was ignored by staff. When asked what reason did resident need pain medication, resident stated spraining ankle the night when resident got lost in the wooded area near the facility.
2. A review of resident?s discharge document from a local hospital emergency room, dated 7-5-20, resident #1 was treated for a sprained ankle with instruction, ?may take Tylenol as needed for pain?. The facility did not provide documentation of the resident receiving medication/over-the counter medication for pain for the ankle injury sustained during elopement incident on 7-4-20.
3. A review of the resident?s July 2020 medication administration record (mar) included Ibuprofen for ankle pain. Further review of the document noted the medication had a start date of July 28, 2020. The mar did not document the medication being administered. The mar also did not document resident #1 receiving any other pain medication in the month of July.
4. A review of resident?s July nurse?s notes submitted noted resident removing boot, however, no documentation of resident?s refusal of any medications.
