This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on 10-30-20. A complaint was received by the department regarding allegations in the area of resident personal funds. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation.
The evidence gathered during the investigation did not support the allegation regarding personal funds. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description: Based on record review and staff interview, the facility failed to ensure it provided to each resident or the resident's legal representative, if one has been appointed, a monthly statement that itemizes any charges made by the facility and any payments received from the resident or on behalf of the resident during the previous calendar month and shall show the balance due or any credits for overpayment. The facility shall also place a copy of the monthly statement in the resident's record.

1. During the remote complaint allegation, a review of resident #1's uniformed assessment instrument (uai) dated 5-24-20, documented the resident having a legal guardian. This information is not documented on the resident's individualized service plan (ISP) dated 6-25-20.
2. Interview with staff #1, staff stated not knowing resident had a legal guardian. Staff also stated resident's legal representative did not receive monthly statements.
3. Resident was admitted to the facility on 5-24-20, and monthly statements of charges and payments were not sent to resident #1's representative noted on the resident's uai. Staff #1, stated not sending statement to representative as the facility is the resident's representative payee..
