Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on 2/16/2021 and concluded on 2/25/2021. A complaint was received by the department regarding allegations in the areas of resident care and reporting. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation.

The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standards or law.

Based on record review, the facility failed to ensure that the fall risk rating shall be reviewed and updated after a fall.

EVIDENCE:

1. The most current fall risk rating was dated 7/21/2020; however, progress notes for resident 1 indicate that she had fallen on 11/23/2020.

Plan of Correction: Administrator or designee will review and update fall risk rating at least annually, when the condition of the resident changes, and after a fall.

Based on record review, the facility failed to ensure that the comprehensive individualized service plan (ISP) shall include a description of identified needs and date identified based upon current physician?s orders.

EVIDENCE:

1. The ISP, dated 7/21/2020, indicated that the vital signs for resident 1 shall be recorded once monthly; however, signed physician?s orders, dated 1/6/2021, indicated that effective 12/5/2020, a pulse oximeter reading and the temperature for resident 1 shall be checked every day, and if a temperature of 99 degrees Fahrenheit and an O2 reading of less than 91% results, then nursing staff shall be notified.
2. The ISP, dated 7/21/2020, indicated that the weight of resident 1 shall be recorded once monthly; however, signed physician?s orders, dated 1/6/2021, indicated that effective 1/6/2021, the weight of resident 1 shall be recorded weekly for four weeks.

Plan of Correction: Administrator or designee will ensure that care and services in the individualized service plan are provided and shall include a description of identified needs and date identifying current physician's orders.
