Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 03-15-2021 and concluded on 03-16-2021. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census was 24. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed 3 resident records, 3 staff records, criminal background checks and sworn disclosures of newly hired staff, staff schedules, fire drills, fire and health inspection reports, dietary oversight, and healthcare oversight.

Information gathered during the inspection determined non-compliances with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-73-70-A
Description: Based on record review and interview, the facility failed to report to the regional licensing office within 24 hours of any major incident that has negatively affected or that threatens the life, health, safety, or welfare of any resident.
Evidence:
1. Resident #1?s ?Fall Incident Report? form dated 01-06-2020 documented, ?? Resident stated [resident] fell off [resident] bed due to being too close to the edge? Minor swelling was visible in areas of the R. hand?? The resident?s ?Individualized Resident Oversight Summary? form dated 10-28-2020 documented, ?? fall- fx 5th rt. [right] hand metacarpal shaft fx [fracture]?? The regional licensing office did not receive an incident report from the facility regarding this incident.
2. Staff #1 stated ?DSS was not notified of fall.? Staff #1 acknowledged the facility did not report the aforementioned incident to the regional licensing office.

Plan of Correction: The facility administrator or his designee will ensure that a report of any major incident that has negatively affected or that threatens the life, health, safety, or welfare of any resident is sent to the regional licensing office within 24 hours.

Standard #: 22VAC40-73-650-E
Description: Based on record review and interview, the facility failed to ensure the resident?s record contained the physician?s or other prescriber?s signed written order.
Evidence:
1. Resident #2?s [home health] ?Direct Care Staff Notes? dated 12-19-2020 documented, ?SN [skilled nurse] visit for SOC [start of care] Assessment and skin assessment??
2. Staff #1 could not provide documentation of a physician?s or prescriber?s signed home health order for skilled nursing.
3. Staff #1 stated the physician provided a written order to the home health agency and acknowledged the facility did not obtain a copy of the physician?s or prescriber?s home health order.

Plan of Correction: The facility administrator or his designee will ensure that the physician?s or other prescriber?s signed written order for all home health services is obtained and placed in the resident?s file.
