Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 04/16/2021 and concluded on 04/29/2021. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census. The inspector emailed the Administrator a list of items required to complete the inspection. The inspector reviewed 4 resident records, 4 staff records.

The information gathered during the inspection determined violations and can be found within the violation notice.

Violations:
Standard #: 22VAC40-73-210-B
Description: Based on Record the facility failed to ensure all direct care staff attend 18 hours of training annually.

Evidence: While reviewing staff #4's training record on 04/16/2021, the training record only included 13.25 hours of training annually.

Plan of Correction: It is Spring Arbor of Williamsburg?s policy to comply with all state regulations regarding direct care staff training. The staff?s training hours will meet or exceed the required hours of annual training for direct care staff. The community?s Business Office Manager, Executive Director, and/or designee, will audit all direct care staff training hours and scheduled training to ensure compliance with meeting the hours of training required.
