Comments:

A initial inspection was initiated on 06-24-2021 and concluded on 06-24-2021. The Administrator was contacted by telephone to initiate the inspection. The Administrator reported that the current census was 51. The inspector emailed the Administrator a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed criminal background checks submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on 06-24-2021. An exit interview was conducted with the Administrator on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:

Standard #:	22VAC40-73-860-G
Description:	Based on observation and interview, the facility failed to ensure hot water taps available to residents were maintained within a range of 105?F to 120?F. Evidence: 1. During the tour of the facility with staff #1 and staff #2, the hot water taps sampled in three out of the six bathrooms were not maintained within the required range: 134.1?F (Room #103), 149.5?F (Room #201), and 124.7?F (Room #402). Steam was observed coming from the stream of hot water. 2. Staff #2 stated hot water temperatures ?usually range from 120?F to 140?F.? 3. Staff #1 and staff #2 acknowledged the aforementioned hot water temperatures were not maintained within the required range in rooms #103, #201, and #402.
Plan of Correction:	The hot water thermostats in all 4 neighborhoods have been set to 115 degrees. Continued water temperature checks will be continued and monitored to ensure compliance with regulations.
