Comments:

A non-mandated complaint inspection was initiated on 6/28/21 and concluded on 7/15/21. A complaint was was received by the department regarding allegations in the area of: Resident Care and Related Services. The licensing inspector conducted on-site observations at the facility on 6/28/21 and 7/15/21. The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and a violation was issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description:	Based on record review and interview, the facility failed to ensure that a preliminary plan of care is developed, on or within seven days prior to the day of admission. Evidence: Resident #1 was admitted to the facility on 5/24/21. The service plan included in Resident #1's record was dated 5/27/21. No other service plans were observed. Facility staff reported that an assessment of resident needs was completed, but the plan in the record was the first service plan.
Plan of Correction:	Going forward, if the comprehensive plan of care is not completed on the day of admission, a preliminary plan of care will be developed by the ED or designee on all new residents on or before the seventh day of admission to cover the basic needs of the resident that adequately protects his or her health, safety and welfare.
