Comments:

A monitoring inspection was initiated on 08/02/2021 and concluded on 08/05/2021. The administrator was conducted by telephone to initiate the inspection. The administrator reported that the current census was 26. The inspector emailed the administrator a list of items required to complete the remote documentation portion of the inspection. The inspector reviewed three resident records, three staff records, the activities schedule, the work schedule, and fire and health inspection documents submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on 08/05/2021. An exit interview was conducted with administrator on the date of the inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Based on observations made during the tour of the building, the facility failed to ensure that all buildings shall be well-ventilated and free from foul, stale, and musty odors. EVIDENCE: 1. The downstairs bathroom in House #1 had a strong smell of urine.
Plan of Correction:	Housekeeping staff will clean and disinfect bathroom frequently per shift to eliminate and prevent urine odor. [sic]
