22VAC40-73 RESIDENT CARE AND RELATED SERVICES

Comments:

A non-mandated self-report inspection was initiated on 10/27/2021 and concluded on 11/04/2021. A self-reported incident was received by the department regarding allegations in the areas of resident care. The Director of Nursing was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Standard #:	22VAC40-73-460-D
Description:	Based on document review and an interview, the facility failed provide attention to specialized needs for prevention of falls. EVIDENCE: 1. An email from the facility on 10/21/2021 indicates resident 1 was assisted to the bathroom on 10/20/2021 at 1:20pm by staff 1 and "staff member put the resident on the toilet, at that moment staff heard another resident's alarm go off and staff 1 left resident on the toilet to check on another alarm. Resident had a fall and was sent out by 911 with a diagnosis of a left hip fracture." 2. The Uniform Assessment Instrument for resident 1 ,dated 08/09/2021 indicates resident requires physical assistance of staff to toilet. 3. The ISP for resident 1, 08/09/2021 indicates "resident will have the assistance of one nursing staff and will utilize mechanical devices such as chair arms, walker, and grab bars to assist to toilet every two hours." and resident "will remain free from falls and injury."
Plan of Correction:	Resident (1) was left on toilet by staff. Resident had fall and was sent out and admitted for left hip fracture. The ISP and UAI updated on 08/08/2021 indicate resident to have assistance with toileting with one staff person. On 10/21/2021, facility provided education to staff member involved in incident. Facility provided same education to all other clinical staff members. Education was provided, including instructions on staying with patient if one or two person assist when toileting. If staff need to step away, they must have another staff member relieve them prior to walking away. Check proper functioning and placement for all alarms. On 11/12/2021, education provided to clinical staff on ISP/Care plans. Each resident has a ISP/Care plan in place that will guide the care needed. From the care plan, tasks will be generated to Point of Care for ADLs that direct the staff on specific needs. It is a description of the action to be taken to meet the needs of the resident and is required to be reviewed during shift.
