A non-mandated complaint inspection was initiated on August 24, 2021 and concluded on October 25, 2021. A complaint was received by the department regarding allegations in the area of monthly charges, resident rights, and the physical plant. The administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the administrator a list of documentation required to complete the investigation. The licensing inspector conducted an on-site observation at the facility on August 26, 2021.
The evidence gathered during the investigation supported some, but not all of the allegation(s) of non-compliance with standards or law, and violations were issued. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 10 calendar days. Please specify how the violation will be corrected. The plan must contain: 1) step(s) to correct the non-compliance with the standard(s), 2) measures to prevent the non-compliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Description: Based on a review of facility documentation and an interview with staff, the facility failed to place a copy of the monthly statement in the resident's record that itemizes any charges made by the facility and any payments received from the resident or on behalf of the resident during the previous calendar month and shall show the balance due or any credits for overpayment.

Evidence: The facility did not place a copy of the monthly statement of itemized charges or payments in the residents' records. Staff #1 stated that the facility maintains a resident account ledger that documents monthly charges and payments received and confirmed that a copy of the monthly statement is not placed in the resident record.

Plan of Correction: Administrator will ensure that monthly statements are placed in resident records.

Description: Based on an interview with staff, the facility failed to ensure that any resident of an assisted living facility has the rights and responsibilities as provided in 63.2-1808 of the Code of Virginia.

-The Rights and responsibilities of assisted living facilities in the 63.2-1808 Code of Virginia states that each person that becomes a resident of an assisted living facility is treated with consideration as a person of worth.
-The facility has 12 bathrooms on the first floor and seven bathrooms on the second floor that are located between two resident rooms (Jack and Jill bathrooms).
-Staff # 1 stated that the bathrooms located between resident rooms are kept locked to prevent residents from damaging the bathrooms.

Plan of Correction: Facility will open additional bathrooms.

Description: Based on observation, the facility failed to ensure that it was free from foul, stale, and musty odors.

Evidence: Upon entering the facility at approximately 10:05 a.m., the licensing inspector noted an odor of urine in the hallway.

Plan of Correction: Administrator has increased housekeepers' hours to prevent odors.
