Comments:
An unannounced mandated monitoring inspection was conducted on March 11, 2022 by a Licensing Inspector and a Licensing Administrator. The facility?s census was 19. The inspection included a review of the building and grounds, mandatory postings, review of health care oversight, fire inspection, health inspection, fire and emergency drills, and staff and resident records. Violations were discussed throughout the inspection and during the exit interview.

Information gathered during the inspection determined non-compliances with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-73-160-A
Description: Based on record review, and staff interview, the facility failed to ensure the administrators attended at least 20 hours of training related to management or operation of a residential facility for adults or relevant to the population in care annually.

Evidence:

1. On 3/11/22, during the review of Staff #1's training records, the record included 13 hours of annual training. Staff #2's training record included 8.5 hours of annual training.

Plan of Correction: Please contact licensing inspector for more information.

Standard #: 22VAC40-73-870-A
Description: Based on observation made during the physical plant inspection, the facility failed to maintain the interior and exterior of the building in good repair and keep it clean and free of rubbish.

Evidence

During a tour of the facility, the following areas were observed to be in need of repair:

1. Resident room #12, the faceplate near the resident bed is dislodging from the wall;
2. Old medical equipment (wheelchair and rollator walker) were stored on the back porch;
3. Down tree branch in the backyard in the resident enclosed sitting area;
4. Rusty outdoor furniture (4 chairs) in the back yard in the resident enclosed sitting area;
5. Resident room #5 there was missing hardware (knobs) from a dresser; and
6. Small hole in the sheetrock near the faceplate near the front entrance.
