Comments:

Type of inspection: Monitoring The licensing inspector was on-site at the facility on 5/16/2022 from 11:10a ? 1:40p. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. There were 95 residents present at the facility during this inspection. The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. The facility was clean. There was construction was taking place in the back exterior of the building. Additional Comments/Discussion: The inspector reviewed five resident records, five staff records, fire and health inspections, health care oversight, dietician reviews, individualized service plans, medication administration was observed, staff schedules and training submitted by the facility to ensure documentation was complete. Documentation was accessible, thorough and well organized. Residents were observed participating in activities and positively interacting with staff. The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
