Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: LI entered the facility on 6/16/2022 at 9:45 am and exited at 12:15 pm on 6/16/2022. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Self-reported incidents were received by VDSS Division of Licensing on 4/29/2022, 5/26/2022, and 6/5/2022 regarding allegations in the resident care and related services.
