Description:	Based on record review and interview, the facility failed to ensure that for each resident with an inability to use the signaling device, in addition to any other services, once the resident has gone to bed each evening until the resident has arisen each morning, at a minimum, direct care staff make rounds no less often than every two hours, except that rounds may be made on a different frequency if requested by the resident and agreed to by the facility. Evidence: 1. Resident 3 had an Individualized Service Plan (ISP), stating that ?Resident 3 cannot use their call button. They will be checked on every 2 hours during rounds so their care needs are met.? 2. The facility?s 2-hour rounding logs were completed on 08/01/2025, 08/11/2025, and 08/12/202. No additional dates for August 2025. 3. Staff 1 and 2 confirmed the information.
Plan of Correction:	While every 2 hour rounds were completed for residents who could not use the call buzzer, the rounding logs had not been completed for the dates mentioned in the violation notice. Manager will ensure rounding logs are completed for those residents who cannot use their call buzzers and check them weekly. Administrator will do spot checks to ensure they are completed.
